Each Participant that is involved in fishing the tournaments needs to fill out the following each year in order to be apart of Ultimate High School Fishing there is a new packet option that has all forms together or you can print them individually.
Volunteer Boat Captain
Boat Captain need to Fill out the following one time per year that they plan on participating.
Tournament Entry Form
Entry forms are required to be filled out for every tournament and turned in the Tuesday before the Tournament Date.
